The following forms are provided to make your first visit easier. Having these forms completed will help expedite your check-in process. We ask that you please arrive 10 minutes prior to your appointment time on your first visit.

• New Patient Form
• Patient Consent Form

Print from your computer and fill out the form. Please bring or email the New Patient Form form with you at the time of your first visit to facilitate processing.

As of April 14, 2003 it is required for all patients to understand their rights under the Health Insurance Portability and Accountability Act of 1996.  To comply with federal regulations, you may view our HIPAA Privacy Notice in our clinic.
